Hit the ball out of the park with this unique educational offering!  This is your opportunity to gain the most up-to-date tools and strategies to support quality care of persons with Alzheimer’s disease and dementia types. Join us for a two day agenda to learn what the latest research is saying about brain health, when to make meaningful connections with different communication approaches, and how to create a dementia friendly and collaborative community.

Upon completion of this program, you should be able to:

• Identify communication approaches that are effective through the stages of the disease.
• Implement strategies for healthy living to stave off cognitive decline.
• Define the role of collaboration to accomplish best care practices and create a dementia friendly community.

 Faculty:

Elaine Jurkowksi, PhD, was previously an Assistant Professor at the University of Windsor, Windsor, Ontario, Canada. She has held administrative and research positions for The Illinois Public Health Leadership Institute at the University of Illinois at Chicago. Dr. Jurkowski began her social work career working with people who have cognitive and developmental disabilities and supporting these people to develop community social, recreation and leisure skills. Her philosophy of “community integration” and “citizen participation” evolved into vocational and residential settings, and included case management and administrative positions in vocational rehabilitation, and residential settings, serving people with disabilities for the Department of Health and Family Services, Province of Manitoba. Dr. Jurkowski was a “Great Cities” Research Fellow in Health and examined strategies for community needs assessment in the United States and Canada, using citizen participation and traditional needs assessments. She has also served as a program evaluation consultant in a range of public and private settings in developed and developing contexts to include countries such as Canada, the United States, India, Niger and Hong Kong.

Yolanda Simon, MS, retired from teaching public school after 36 years where she enjoyed being in the classroom, and working in administration.   She brings her passion of meeting and talking to new people to the Alzheimer’s Association where she volunteers as a Community Educator and Community Representative.  She delivers education programs throughout the community, as well as connecting people with resources they might need.  Yolanda, who was a caregiver to her mother while she was battling Alzheimer’s disease for 10 years, is passionate about helping those in similar situations.

Jane Williams, CDP, is currently the administrator at Prairie Living of Chautauqua located in Carbondale.  She has 8 years’ of experience with assisted living administration and has been a Certified Dementia Practitioner for 9 years.  She has been a volunteer with the Alzheimer’s Association for 6 years.  Her roles include, Community Representative, Community Educator and Ambassador.  Jane has a real passion for the elderly and is committed to the mission of the Alzheimer’s Association both personally, and professionally.
